First-Time Patient Information

Your first visit will be approximately 1 hour in duration.  We ask that you arrive 15 minutes before your appointment time to complete the paperwork.

Please bring with you:

• Your driver’s license
• Your insurance card(s) – if you wish us to bill insurance
• A list of medications/vitamins/supplements you are taking and their dosages
• Your physician’s referral/physical therapy prescription – if you have one
• Completed paperwork

Please dress comfortably for the visit.

Rescheduling

We understand that life can happen.  If you need to reschedule or cancel your appointment, please contact us at least 24 hours before your scheduled appointment time.

Payment and Insurance

We accept ALL insurance including, but not limited to:

• EBMS
• New West
• Workers Compensation
• Auto Insurance

We will work with your insurance to ensure timely and proper payment.  In most cases, your insurance will cover Physical Therapy with a prescription/referral from your physician.  Not all insurance plans requite a physician’s prescription/referral; please check with your insurance provider for the specific requirements of your plan.
